USMLE Application Form 2021 - Before candidates start filling USMLE 2021 application form, they must note that the United States Medical Licensing Examination (USMLE) is made up of three steps: Step 1, Step 2 CK, Step 2 CS, and Step 3. The candidate may take Step 1, Step 2 CK, Step 2 CS in any sequence, but Step 3 is allowed only after passing all previous steps. USMLE Registration and Test Delivery

The registration process for USMLE steps has to be completed through the appropriate organization. For example, if the candidate is taking Step 1 and Step 2, he/she has to register through ECFMG. Alternatively, FSMB is the registration authority for Step 3; check below for full details.

USMLE Step 1 and Step 2 Registration

ECFMG is the organization that registers international medical students/graduates for Step 1 and Step 2 (CK and CS).
ECFMG will be responsible for processing USMLE 2021 application form and payment, verification of eligibility, and sending notifications regarding the outcome of application.
For eligible international medical students/graduates applying for Step 1/Step 2 CK, ECFMG is responsible for forwarding registration information to NBME, who issues exam scheduling permits.
ECFMG will send e-mail notifications to applicants upon availability of scheduling permits.
Prometric is responsible for scheduling and delivering test centers for USMLE Step 1 and Step 2 CK. It acts as the test delivery entity for all examinees taking Step 1/Step 2 CK. Step 1 and Step 2 CK can be taken at Prometric test centers across the world.
For eligible international medical students/graduates applying for Step 2 CS, ECFMG is responsible for issuing exam scheduling permits and sending e-mail notifications to applicants once scheduling permits are ready. The Clinical Skills Evaluation Collaboration (CSEC), a joint venture of ECFMG and NBME, conducts the Step 2 CS exam. Step 2 CS is delivered to all examinees at regional CSEC Centers in the United States.
NBME determines the results of USMLE exams for all applicants and issues score reports. ECFMG will send test takers e-mail notifications when their Step 1, Step 2 CK, and Step 2 CS score reports are ready.

Note: The NBME is the registration entity for students/graduates of U.S. and Canadian medical school programs accredited by the Liaison Committee on Medical Education (LCME) and U.S. medical schools accredited by the American Osteopathic Association (AOA).

USMLE Step 3 Registration

The Federation of State Medical Boards (FSMB) is the organization that registers all Step 3 applicants.
Also for all applicants, NBME is responsible for determining the results of USMLE exams and for issuing the score reports. FSMB notifies examinees when their Step 3 score reports are available.

Application for USMLE Step 1, Step 2 CK, and Step 2 CS would comprise:

Submission of on-line application
Student or graduate status verification by the medical school
Any other required documents, as outlined in IWA and the ECFMG Information Booklet.

How to apply for USMLE Step 1, Step 2 CK, and Step 2 CS

Candidates can apply for USMLE Step 1, Step 2 CK, and Step 2 CS irrespective of whether they are applying for ECFMG for examination for the first time or have applied before.
ECFMG will process USME 2021 application forms after receiving the on-line part of the exam application and verifying the candidate's student or graduate status from the medical school. It will also verify and other documentary requirements.
To complete the USMLE 2021 application for these steps, the candidate will need a valid email id and an USMLE/ ECFMG Identification number and an on-line services password.
The candidate will also have to select an acceptable payment method.
The on-line application doesn't have to be completed in one session, but it must be completed within 14 days from the date it was last saved. Or else the application will no longer be valid and a new application has to be filled.

USMLE Status Verification

ECFMG will verify the applicant's status as a currently enrolled student or graduate directly from his/her medical school. Only if the verification is successful, the candidate will be able to register for USMLE. ECFMG will send status verification request to the medical school as per the school's preferred method:

Electronically through the ECFMG Medical School Web Portal (EMSWP) Status Verification program, or
Via a paper form (Certification Statement Form 183).

If the medical school completes the status verification electronically, the candidate will be notified about it after the completion of the on-line application. However, if the verification is done in paper form, the candidate will be provided with the form at the end of the on-line application process.

Information related to credentials that has to be submitted at the time of application will be notified as the candidate fills out the exam application.

USMLE Step 3 Application

Step 3 is the final step in the USMLE sequence completion of which would lead to a license to practice medicine in the United States without supervision. Step 3 is the final assessment of physicians before they assume independent responsibility to deliver general medical care. It must be noted that Step 3 cannot be taken in the first two weeks of January or on major holidays. Test dates will be allotted on a first-come, first-served basis, and the USMLE program doesn't guarantee availability of test centers. Therefore, the candidate should contact Prometric as soon as possible after receiving the scheduling permit to schedule test dates.

How to apply for USMLE Step 3

All medical school graduates who have passed Step 1, Step 2 CK, and Step 2 CS are eligible to take Step 3
USMLE application form for Step 3 has to be submitted through FSMB website.
The candidate has to submit a Certification of Identity (CID). The CID application is mandatory for all Step 3 applications, unless a valid one is already on file.
Approved CIDs from prior Step 3 applications are valid for a period of 5 years.
However, the submission of CID is not considered an application for Step 3
The candidate instead has to submit an on-line Step 3 application and make the exam fee payment
The application fee for Step 3 is non-refundable and non-transferable from one eligibility period to another or from one application to another. Further, the application fee can only be paid using American Express, MasterCard or Visa.
Any request for test accommodations has to be made in writing at the time of application submission for Step 3.

Medical School Students

Condition 1:

The applicant must be officially enrolled in a medical school located outside the United States and Canada that is listed in the World Directory as meeting ECFMG eligibility requirements, both:

At the time that of application, and
On your test day.

Also, the "Graduation Years" in the ECFMG note in the applicant's medical school's World Directory listing must be "Current" at the time of application and and on test day.

Condition 2:

In addition to being currently enrolled,the applicant must have completed at least two years of medical school.

Condition 3:

Medical school students who take a leave of absence should consult with their medical schools about whether they will be considered officially enrolled in medical school during leave.

If the medical school considers a student on leave of absence to be withdrawn from medical school, the student is not officially enrolled in medical school and so is not eligible to apply for or take USMLE.

Medical School Graduates

Condition 1:

The physician must be a graduate of a medical school located outside the United States and Canada that is listed in the World Directory as meeting ECFMG eligibility requirements.

The graduation year must be included in the "Graduation Years" listed in the ECFMG note in the medical school's World Directory listing.

Condition 2:

The candidate must have been awarded credit for at least four credit years (academic years for which credit has been given toward completion of the medical curriculum) by a medical school that is listed in the World

Directory as meeting ECFMG eligibility requirements.

Condition 3:

An authorized official from the medical school must certify the candidate's status as a graduate of the school; instructions can be checked at the time of application for USMLE examination.

Meanwhile, those appearing for Step 3 must fulfill the following conditions:

To be eligible for Step 3, international medical graduates must have passed Step 1 and Step 2 (CK and CS)
And must be certified by ECFMG, among other requirements.

What is ECFMG Certification?

The Educational Commission for Foreign Medical Graduates (ECFMG) certification assesses whether international medical graduates are fit to enter residency or fellowship programs in the United States that are accredited by the Accreditation Council for Graduate Medical Education (ACGME).

ACGME mandates that international medical graduates who enter ACGME-accredited programs are certified by ECFMG.

Further,

ECFMG Certification is also one of the eligibility requirements for international medical graduates (IMGs) to take Step 3 of the three-step USMLE

ECFMG Eligibility Criteria

Medical School Requirements: The applicant's medical school must be listed in the World Directory of Medical Schools (World Directory) as meeting ECFMG eligibility requirements.
Examination Requirements: The physician must have passed Step 1 and Step 2 of the USMLE. The Step 2 exam comprises two separately administered components: the Clinical Knowledge (CK) component and the Clinical Skills (CS) component.
Time limit for completion of examination requirements: As per ECFMG policy, international medical students/graduates must pass USMLE Steps and Step Components within a seven-year period. The seven-year period begins on the exam date of the first Step or Step Component passed and ends exactly seven years from that exam date.
